How to Track Your Workouts: Sets, Reps, Weight and Notes

For each exercise, record what you actually did: the exercise, your working sets, the reps completed in each set, and the weight or resistance used. For most resistance-training sessions, that is enough to create a useful training history.
Add a note when something meaningfully changes how the result should be interpreted — a different machine, unusually short rest, a failed rep, a technique change, or anything else you will want to remember next time. Your programme describes what you intended to do; your workout log should preserve what actually happened.
The goal is not to collect every metric available. It is to record enough that you can understand the session later, compare it fairly with previous workouts, and make a sensible decision next time.
What should you record in a workout?
Start with four things: the exercise name, your working sets, the completed reps in each set, and the weight or resistance used.

That is more useful than writing Bench press — 60 kg, 3 × 10 if 3 × 10 was only the target. If all three sets genuinely were 10 reps, shorthand such as 60 kg — 3 × 10 is perfectly fine.
A programme might prescribe three sets of 8–10 reps, but that only tells you what you were trying to do. If you completed 10, 9 and 8 reps, preserve 10, 9 and 8. Next time, you can see exactly where you left off and decide whether to add a rep, repeat the performance or eventually increase the load.

How to record sets, reps and warm-ups
Record repetitions you actually completed through the range of motion you intended to use. If you complete eight good reps and cannot finish the ninth, the result is eight reps; if the failed attempt matters, write 60 kg × 8 — failed 9th. Likewise, a partial can be preserved as 80 kg × 7 + 1 partial rather than silently counted as a normal completed rep.
You do not need a universal notation system. You need one that still makes sense when you look at the workout weeks or months later. If several sets genuinely match, compress them:

Warm-up sets can also be worth recording, particularly as your working weights increase or when the warm-up itself changes. Keep them clearly distinguishable from the sets you use to judge performance:

You do not need to record every warm-up forever if it follows the same predictable pattern and adds no useful information. In HyperResponder, warm-up sets can be tagged separately and are excluded from workout volume, strength PRs and estimated-1RM calculations.
How to log AMRAPs, supersets and drop sets
Unusual set types do not require a different logging system. Preserve the actual result and enough context to understand it later.

AMRAP describes the instruction; 13 reps is the result. A superset should keep the individual exercises identifiable, while a drop set should preserve each load and the reps completed at that load.

Normal working sets can contribute to volume, PRs and estimated 1RM; warm-ups are excluded from all three, while drop-set work contributes to workout volume but the fatigued dropped portions are excluded from strength PR and estimated-1RM calculations.
How to record weight
Consistency matters more than finding the one notation every lifter uses. If the same number means something different from one session to another, your history becomes difficult to compare.
Barbells. Record the total weight being lifted, including the bar. Bench press — 60 kg × 10 should mean 60 kg in total, not 60 kg of plates plus the bar. This is also the convention HyperResponder expects.
Dumbbells. Record the weight of one dumbbell: Dumbbell bench press — 22.5 kg × 10 means a 22.5 kg dumbbell in each hand. HyperResponder follows this convention and accounts for both dumbbells when calculating total load, so you can enter the number printed on the dumbbell rather than doubling it yourself.
Unilateral exercises. If both sides perform the same work, a compact entry such as Single-arm lateral raise — 8 kg × 10 each side is enough. If your own notation consistently means both sides, 8 kg × 10 can also be sufficient. Record the sides separately only when they differ.
Bodyweight exercises. For an ordinary bodyweight movement, reps may be all you need to enter: Pull-ups — 8, 7, 6. HyperResponder can incorporate your bodyweight into the load used for these movements. For weighted pull-ups, make the added load explicit (+10 kg × 6); for assisted work, record the assistance (20 kg assistance × 8).
Machines and setup. Two machines displaying the same number are not necessarily equivalent. Record a machine, seat position, attachment, grip, stance or range-of-motion change when it meaningfully affects the comparison, not simply because the setting exists.
What should you write in workout notes?
A useful note explains something the numbers cannot: why performance changed, whether the exercise setup was different, what affected a set, or what you need to remember next time.

You do not need to write a diary after every workout. If the numbers tell the full story, the numbers are enough.

That preserves the exercises, load, reps, warm-up context, individual working-set results and one useful instruction for next time. You do not need much more than that to build a useful training history.
Do you need to track RPE, RIR, rest and tempo?
Not necessarily. Exercises, working sets, reps and load give you a useful foundation; extra metrics are worth adding when your programme uses them or when they help you make a decision.
RPE and RIR can add context around effort. Squat — 100 kg × 5 @ 2 RIR means you estimate that roughly two more repetitions were available. These ratings can be useful, but they are subjective estimates rather than requirements for a useful log.
Rest time matters when it changes performance. If you normally rest three minutes but only had 60 seconds before a poor final set, 100 kg × 3 — only 60 sec rest gives the result useful context. You do not need to time every rest period unless rest itself is something you are trying to control.
Tempo is worth recording when it is prescribed or deliberately changed. 60 kg × 8 — 3 sec eccentric may describe a meaningfully different performance from your normal technique; if your tempo remains consistent, another field is unnecessary.
Bodyweight, machine settings, sleep, soreness and session duration can also be useful in the right circumstances. Add them when they help answer a training question rather than because a tracker gives you somewhere to put them.
How to use your workout log to track progress
Before repeating an exercise, look at your previous comparable performance: the load, reps per set, number of working sets and any note that changes how the result should be interpreted. Then compare that history with the target in your programme.
Progress might mean more reps at the same weight, more weight for similar reps, reaching the top of a prescribed rep range, completing another programmed set, using less assistance, improving technique or simply returning to previous performance after time away. It does not have to mean adding weight every workout.
The comparison only works if you are comparing reasonably similar performances. A paused bench press and a touch-and-go bench press may both say 60 kg × 8, but they are not necessarily equivalent. The same applies when the machine, range of motion, rest interval, equipment setup, bodyweight or set type changes.
You do not need laboratory-standard consistency. You just need enough context to recognise when two numbers are not directly comparable.
What about workout volume and estimated 1RM?
Once your workouts are recorded consistently, secondary metrics can help describe your training.
A simple volume-load calculation is weight × completed reps. A set of 60 kg × 10 therefore contributes 600 kg of volume load. Summing the relevant sets can describe how much loaded work you performed, but more volume does not automatically mean a better workout or greater training effect.
Estimated one-rep maximum calculations use a submaximal set to approximate maximal strength. One commonly used example is the Epley equation: estimated 1RM = weight × (1 + reps ÷ 30). Treat the result as an estimate rather than a tested maximum; different exercises, repetition ranges and equations can produce different values.
For everyday training, the trend across comparable sessions is usually more useful than obsessing over one calculated number. This is also why correct set classification matters: a warm-up or fatigued drop-set portion should not be treated as though it were an ordinary strength test.
Paper, spreadsheet or workout app?
There is no single correct way to track workouts. Paper is simple and flexible but becomes harder to search as your history grows. A spreadsheet offers tables, formulas and charts but can be awkward during a live workout. A dedicated tracker can organise exercise history, previous loads and reps, programmes, PRs and progress automatically, although some interfaces require a lot of set-by-set interaction.
The best method is the one that captures accurate information with little enough friction that you continue using it.

Common workout-tracking mistakes
Logging the target instead of the result. If your programme says
3 × 10but you completed10, 9, 8, record10, 9, 8.Counting failed reps as completed. Preserve a failed or partial attempt as context rather than silently turning it into a successful rep.
Changing exercise names constantly. Inconsistent naming can fragment the history of the same movement.
Treating different variations as identical. A paused bench, touch-and-go bench and machine chest press should not automatically share the same comparison.
Mixing weight conventions. Do not record dumbbells per hand one week and combined weight the next, or alternate between total barbell weight and plates only.
Mixing warm-ups with working sets. Warm-ups can be worth recording, but keep them distinguishable from the sets you use to judge performance.
Tracking too much or never reviewing it. Exercises, sets, reps, load and useful context are enough to start; the history becomes valuable when you actually use it to inform the next workout.
What if you forget to log a workout?
Log it later if you can accurately remember what you did. A backdated workout is more useful than a missing session when it reflects what actually happened, so use the date on which the workout took place rather than pretending it happened today.
If you later notice a genuine entry error — the wrong weight, exercise or rep count — correct it. Your training history should become more accurate when you fix a mistake, not preserve the mistake forever.
A simple workout-tracking checklist
Before you finish logging, check that:
the exercises are recorded;
the working sets reflect what you actually completed;
the reps are accurate;
the weight convention is clear;
warm-ups and unusual sets are distinguishable where necessary;
useful context has been preserved; and
you can understand the entry without relying on memory.
If the answer is yes, you probably have enough information.
You do not need the most detailed workout log. You need one that you can maintain, understand later and use when deciding what to do next.
Track what happened, keep it consistent, and add context when it matters. That is enough to turn individual workouts into useful training history.
